7. Marriage Certificate, if married

Transferees

1. Original Copy of Transfer Credential ( Honorable Dismissal)
2. Transcript of Record for Evaluation or a Copy of Scholastic record showing the subjects, number of units taken, and the grades obtained.
3. Baptismal and Birth Certificate ( Original & Photocopy)
4. Good Moral Recommendation

Cross-Enrollees

1. Cross Enrollment Permit from current school.

3. Two (2) photocopies of verified ACR/ICR/Valid Visa for Study.
